Case Management Services for Traumatic Injuries

Ridgemoor Case Management Services specializes in cases involving traumatic brain injury, spinal cord injury, amputation, burns, orthopedic injuries, and work-related injuries.

Evaluate the injured individual's situation and identify their needs and goals.
Collaborate with treatment providers to identify an individualized rehabilitation plan for the injured individual and assist in implementing the plan.
Empower the injured individual to make informed decisions relative to treatment options and provide support.
Coordinate appropriate, high-quality medical and rehabilitation services, including therapy, attendant care, and school/vocational services as needed.
Address needs include durable medical equipment, medications, home modifications, alternative housing, transportation, and vehicle modifications.
Serve as a communication bridge between the injured individual, their family, treatment providers, and the insurer.
Update the rehabilitation plan as the injured individual progresses and their needs change.
Promote open communication with the insurer regarding plan of care to facilitate coverage for needed items and services.

Available Positions

We have a position available for the right person working primarily with clients who have sustained a traumatic brain injury, spinal cord injury, and other catastrophic injuries. Other employment opportunities include research for Life Care Plans and Workers' Compensation case management.

Training and Flexible Hours

In addition to the RN requirement, we provide extensive training and development for additional certifications. Flexible hours with work-from-home potential.
